an unexpected crisis occurs in an older person's life, chaos may follow. When
a person has a long term illness or disability, their life and the lives of their
family can be turned upside down. Embrace, Elder Care Solutions knows how to lessen
the stress of these situations.
Embrace
members can be sure that
help will be there when it is needed.
No
one can predict what the future will bring, but we have some clues. Our staff
of geriatriatric professionals has the experience and knowledge to help you through
the rough times, if they should come.
Our
goal is to ensure a rapid response to any future hospitalization or other medical
crisis. The coordinator will help you develop a personalized plan ready to be
put into action with one phone call to Jewish Family Service. The Embrace coordinator
will: communicate with
identified out of town or local relatives or friends who need information;link
the Embrace member of their family to JFS programs and other local services to
meet changing care needs; and take steps to assure a smooth transition home following
a medical crisis.
Health
and wellness education
As
a member of Embrace, you will be invited to attend educational programs and will
receive a member newsletter designed to help promote good health and quality of
life.
